In-Home Support Services

The goal of IIH services is to provide preventative treatment to youth at risk of displacement from home environment, or upon reunification after an out of home placement. Family services, Inc’s IIH program will provide various interventions and therapeutic services in a community based setting. Our services support our clients’ mental, emotional and behavioral health needs. We implement diagnostic assessments and evaluations to inform the treatment interventions that create desired outcomes while identifying client needs and the challenges that create potential for out of home placement.

FAMILY INVOLVEMENT

Providers utilize interventions such as individual and family therapy where clients have the opportunity to be engaged in behavioral modification and family intervention to support the client within their natural support system and build family skills for long term success.

CASE MANAGEMENT

Case Management services assists our clients with accessing the services and support that are essential to meeting their basic needs identified in their individualized service plan (ISP). The ISP includes a plan to access needed mental health, mental retardation and substance abuse services, as wall as medical nutritional, social, educational, vocational, and employment, housing, economic assistance, transportation, leisure and recreational, legal, and advocacy services and supports that the individual needs to function in a community setting.

INTERVENTIONS

Activities that contribute to the success of our clients’ mental, emotional, and behavioral health and create a safe space for them to communicate in order to gain better relationships with their support systems.
Services include but are not limited to: Staff work directly in the consumer’s home 1-5 times per week depending on the consumer’s need. Staff provide supportive counseling to the consumer as needed.
